He is also the editorin-chief of Clinical Genitourinary Cancer and the author of more than 100 scientific papers.
Sartor was also the Principal Investigator for the prospective randomized trial that led to Xofigo’s FDA approval in May 2013.
Prostatepedia spoke with Dr. Sartor about Xofigo’s role in treating bone metastases in men with prostate cancer.

we know that it’s true that prostate cancer readily spreads to bone, but the reasons why are debatable.

P. Do some prostate cancer treatments make bone more susceptible to prostate cancer spread?

Dr. Sartor: No, I don’t think that is how I’d perceive it. The bone is a natural target for prostate cancer regardless of therapeutic approach.
There have been studies in which we try to interfere with the bone microenvironment to alter the propensity of cancer cells to traffic to bone. This was tried in particular with Xgeva. There was a very slight improvement in the overall trial result. It was submitted to the FDA, but the results were not adequate enough to declare it clinically significant. Furthermore, with Xgeva use we see a relatively high risk of osteonecrosis of the jaw.
The bottom line at this point in time is that we don’t have any agents that have a propensity to result in bone metastasis. It appears to be part of the natural history of the disease.

P. How and when are bone metastases typically detected? In newly diagnosed men or men with recurrent disease?

Dr. Sartor: It turns out that that is a little bit difficult to ascertain in a quantitative way. If we look at clinical trials that have treated people with bone metastases, we see that about 5% of American men present initially with metastatic disease, the majority of which is in bone. Now, if we look at the current numbers for prostate cancer —220,000 to 240,000 men diagnosed per year—and impose the 5% rule, we’re talking about somewhere between 11,000 or 12,000 men diagnosed with metastatic disease every year, the vast majority of whom have bone metastases.
That accounts for a very substantial proportion. However, the majority of cases probably arise in people who have been treated initially, including with hormones, and then fail— i.e. their cancer starts growing again. These cases probably account for somewhere around 24,000 men
per year. That is a rough estimate. The bottom line is that about 2:1 develop it after initial diagnosis as opposed to at initial diagnosis.
The majority of men today are not diagnosed with metastatic disease. They develop metastatic disease after initial treatment.

Before we started PSA screening in the late 1980s, 20 to 22% of men were being diagnosed with metastatic disease each year.
Today, there are more men being diagnosed overall, but only about 5% have metastatic disease. If we stop PSA testing, I think the total number of cancers diagnosed will go down, but the number of those men with metastatic disease will go up.

The first category of men has hormone-sensitive prostate cancer with bony metastases. These are newly diagnosed men who have normal testosterone levels (200-600 ng/dl). They have not been treated with hormonal therapy yet.
Today, these men are typically treated with androgen deprivation therapy, which is a therapy that uses a drug to lower testosterone levels. New data from two large trials recently reporting, CHAARTED and PREVAIL, suggest that the combination of androgen deprivation therapy plus the chemotherapy drug Taxotere represents the current standard of care for most men diagnosed with metastatic disease who have not had prior hormonal therapy.

The second category of patients is men who have developed bone metastatic prostate cancer and already have low testosterone levels. We define low testosterone levels as less than 50 ng/dl. That is an old definition used by the FDA, but it is still reasonably used today. Testosterone levels less than 50 ng/dl with progression of disease in bone metastasis means that you have something called castrate-resistant prostate cancer, or CRPC.
Men with CRPC disease have a different set of treatment options. There are six FDA-approved drugs shown to prolong survival. They are Zytiga, Xtandi, Taxotere, Provenge, Xofigo, and Jevtana.
There are several FDA-approved agents used to strengthen the bone in an effort to prevent what we call symptomatic skeletal events or skeletal-related events. (Note that these agents are not used to treat the bone metastases themselves nor the associated bone pain.) These agents include Zometa,
which is a bisphosphonate, and Xgeva, another form of an osteoclastinhibiting agent. These agents do not prolong survival, but can decrease skeletal-related events—things like fractures, spinal cord compression, the need for radiation or surgery to bone, etc.

Dr. Sartor: Xofigo is the only bonetargeted therapy that prolongs survival.
We approach prostate cancer today within the domain of life-prolonging therapies. Life-prolonging therapies include Zytiga, Xtandi, Taxotere, Provenge, and Xofigo. We’re considering using Zometa and Xgeva in combination with these five life-prolonging agents going forward. There are various degrees of benefit, depending on the amount of bony metastasis and the condition of a man’s jaw and teeth. (I mentioned that osteonecrosis of the jaw can be a side effect, so we have to check in advance the condition of the jaw and teeth.)
After initial treatment, we typically treat until there is evidence of progression and then bring in another line of therapy. Which therapy we use next is complicated. There is a lot of debate over which sequence is best and how we should use these agents in sequence. Suffice it to say that we know a lot about the Taxotere-treated patient. Jevtana, Xofigo, Zytiga, and Xtandi have all been proven in clinical trials to be effective after Taxotere. But if we use Zytiga or Xtandi before Taxotere, it is a little bit harder to say which sequence is optimal.

We approach prostate cancer today within the domain of life-prolonging therapies. Life-prolonging therapies include Zytiga, Xtandi, Taxotere, Provenge, and Xofigo. We’re considering using Zometa and Xgeva in combination with these five life-prolonging agents going forward. There are various degrees of benefit, depending on the amount of bony metastasis and the condition of a man’s jaw and teeth. (I mentioned that osteonecrosis of the jaw can be a side effect, so we have to check in advance the condition of the jaw and teeth.)
After initial treatment, we typically treat until there is evidence of progression and then bring in another line of therapy. Which therapy we use next is complicated. There is a lot of debate over which sequence is best and how we should use these agents in sequence. Suffice it to say that we know a lot about the Taxotere-treated patient. Jevtana, Xofigo, Zytiga, and Xtandi have all been proven in clinical trials to be effective after Taxotere. But if we use Zytiga or Xtandi before Taxotere, it is a little bit harder to say which sequence is optimal.

Skuteczność Jevtany, Xofigo, Zytigi oraz Xtandi po Taxotere została udowodniona w wielu próbach klinicznych.
Ale jeśli stosujemy Zytigę lub Xtandi przed Taxotere, jest trudniej stwierdzić, jaka kolejność jest opytymalna.
